Delete your DIALIQ account :

Open Dialiq.

Your account will be permanently deleted immediately. This action cannot be undone.

Email doyoungkim@osmiq.from the email address associated with your DIALIQ account and use the subject line:

We will process your request and send you a confirmation email once deletion is complete, normally within a few business days.

What will be deleted

Deleting your DIALIQ account removes the personal information stored as part of your DIALIQ account, including:

• Your DIALIQ account, sign-in information and profile

• Diary entries and meal records

• Fluid and phosphate binder logs

• Symptoms you have recorded

• Blood pressure and weight measurements you have entered

• Saved DIALIQ Chat conversations

• Your dialysis schedule and personal treatment targets stored in DIALIQ

• Your connection between DIALIQ and your clinic

What may be retained

If your DIALIQ account was connected to a clinic, information entered or maintained by your healthcare team, such as pathology results or other clinical records, may form part of your clinic's official medical record.

Deleting your DIALIQ account removes DIALIQ's connection to those records. It does not delete, modify or otherwise affect records held independently by your healthcare provider.

Your clinic will retain those records in accordance with its own legal, regulatory and clinical recordkeeping obligations.
